Committed to the health and well-being of the people of Western North Carolina
CarePartners Foundation is committed to preserving the availability of post-acute healthcare in Western North Carolina by working with donors to fund vital services. We focus on the needs of those who receive care from CarePartners Health Services, which provides Rehabilitation, Home Health, Adult Care and Hospice. We also make grants to community agencies working to improve the health and well-being of people in Western North Carolina. We are a 501(c)(3) not-for-profit organization.
We are thrilled to welcome PACE to the CarePartners continuum of post-acute care. CarePartners PACE (Program of All-inclusive Care for the Elderly) is a program for people age 55 or older that need support to remain safely at home. It provides community-based care and services for people who might otherwise need a nursing home level of care. A PACE program works to promote and sustain the independence of seniors, and help those who want to live at home, stay at home. Click to learn more about PACE.

Special focus on Hospice care
CarePartners Hospice Foundation is a special part of CarePartners Foundation, dedicated solely to meeting the needs of Hospice patients and their families. Every gift designated to Hospice is used to make sure everyone who needs Hospice care receives Hospice care. No one is turned away for lack of ability to pay.
We invite you to join us by making a donation or becoming a volunteer. You can also support the work of CarePartners Foundation through the Hospice Thrift Store, our Estate Sales Services, our Gift Shop and our special events.
